چکیده
The most critical factor in rehabilitation of the geriatric lowerextremity amputee on a prosthesis is preservation of the knee joint. Several reported series have established that most amputations below the knee will heal despite extensive vascular occlusive disease [I-41. Satisfactory healing appears to be a direct result of attention to atraumatic surgical detail, especially in the careful management of skin closure. A new technique for the postoperative management of the amputee has recently been introduced. A temporary prosthesis is applied at the time of operation, which permits limited weight-bearing ambulation by the first or second postoperative day [.5-141. This method offers the

LATE ARTERIAL REPAIR FOR LOWER LIMB OR KNEE SALVAGE
The objective of this report is to prove that even after development of ischemic necrosis of the muscles and nerves, we can still save the extremity or the knee and even several days after arterial injury, perform arterial repair and other mentioned procedures. Thus we can retain a functionally satisfactory leg or knee joint, which is far better than classic above-knee amputation and utili...
متن کاملTherapeutic and economic impact of a modern amputation program.
The experience with 142 below-knee amputations for vascular occlusive disease and/or diabetes mellitus in 133 patients has been reviewed. The program utilized Xenon(133) skin bloodflow measurement for the selection of amputation level, emphasized the use of the long posterior skin flap as an important part of surgical technique, and employed immediate postoperative prosthesis with accelerated r...
متن کاملDebridement of necrotic eschar with 40% urea paste speeds healing of residual limbs and avoids further surgery.
A 50-year-old man with diabetes (patient 1) underwent left below-knee amputation (BKA) for a nonhealing infected foot ulcer. Necrotic eschars developed on the residual limb (Figure 1). He was referred to the dermatology department postoperatively for wound care prior to a planned above-knee amputation (AKA). A 62-year-old man with diabetes (patient 2) underwent bilateral BKA for gangrene second...

Background: Through-knee amputation provides a longer lever arm and improved muscle control of the limb compared with above-knee amputation. Through-knee amputation also allows use of a total end-bearing prosthesis, which avoids the ischial pressure and suspension belts required of the above-knee amputation prosthesis. Several reports in the European literature tout the superiority of the throu...
متن کاملImmediate postsurgical prosthetics fitting in the management of upper-extremity amputees.
1 School of Medicine, University of Miami, Jackson Memorial Hospital, Miami, Fla. 33152. In the experience of the authors of this article, immediate postsurgical prosthetics fitting has been the most satisfactory means of managing lower-extremity amputees (1,2,3,4,5). The procedure has allowed better control of postsurgical edema, reduced postoperative pain, permitted more rapid conditioning of...
